Vietnam is considered one of the 'fast-track countries' to achieve the health-related Millennium Development Goals, but research on its equity strategies remains inadequate.Methods Using Vietnamese official health statistics, we investigated inequity in four dimensions including health resources, service delivery, service utilisation and residents' health status from the perspectives of income levels, poverty rates and subnational regions. The Slope Index of Inequality, concentration curve/Concentration Index, absolute difference and Theil Index were used.Results Four indicators showed 'pro-poor' inequality in health resources, including the per capita health budget, per capita health personnel, per capita health personnel at the community level and per capita hospital beds at the community level, while provincial hospital beds showed 'pro-rich' inequality. Two health service delivery indicators (delivery of antenatal care >= 3 times and proportion of community health service centres with medical doctors) show 'pro-rich' inequality, although two health status indicators, mortality and malnutrition rates for children under five, showed 'pro-poor' inequality. The Northern Midlands and Mountain Areas, and the Central Highlands were disadvantaged regarding service delivery and health status. Intraregional differences were the main factors contributing to the inequalities in delivery of antenatal care >= 3 times, provincial hospital beds and percentage of community health centres with medical doctors, with the Red River Delta and the South East region experiencing the greatest inequalities.Conclusion The overall level of health equity in Vietnam has increased over the past decade, although inequality in health service delivery has hindered progress towards health equity based on income, poverty and subnational regions. Shao, QH; Li, RD; Han, YF; Han, DF; Qiu, J Temporal and Spatial Evolution of the African Swine Fever Epidemic in Vietnam 越南非洲猪瘟疫情的时空演变 INTERNATIONAL JOURNAL OF ENVIRONMENTAL RESEARCH AND PUBLIC HEALTH African swine fever (ASF) is a severe infectious disease affecting domestic and wild suids. Spatiotemporal dynamics analysis of the ASF is crucial to understanding its transmission. The ASF broke out in Vietnam in February 2019. The research on the spatiotemporal evolution characteristics of ASF in Vietnam is lacking. Spatiotemporal statistical methods, including direction analysis, spatial autocorrelation analysis, and spatiotemporal scan statistics were used to reveal the dynamics of the spatial diffusion direction and spatiotemporal aggregation characteristics of ASF in Vietnam. According to the cessation of the epidemic, it was divided into three phases: February to August 2019 (phase 1), April to December 2020 (phase 2), and January 2021 to March 2022 (phase 3). The ASF showed a significant spread trend from north to south in phase 1. The occurrence rate of the ASF aggregated spatially in phase 1 and became random in phases 2 and 3. The high-high ASF clusters (the province was a high cluster and both it and its neighbors had a high ASF occurrence rate) were concentrated in the north in phases 1 and 2. Four spatiotemporal high-risk ASF clusters were identified with a mean radius of 121.88 km. In general, there were significant concentrated outbreak areas and directional spread in the early stage and small-scale, high-frequency, and randomly scattered outbreaks in the later stage. The findings could contribute to a deeper understanding of the spatiotemporal spread of the ASF in Vietnam. 本研究运用时空统计方法分析了非洲猪瘟在越南的传播动态与聚集特征。根据疫情进程分为三个阶段,结果显示,疫情在第一阶段呈现显著的自北向南扩散趋势;空间聚集性在第一阶段明显,而在第二、三阶段转为随机分布。高低聚集区域在第一、二阶段集中在北部地区。研究共识别出四个时空高风险聚集区,平均半径较大。总体来看,非洲猪瘟在越南的传播表现为早期存在明显的集中暴发区和定向扩散,后期则呈现小规模、高频次、随机分散的特点。这些发现有助于深入理解非洲猪瘟在越南的时空传播规律。 2022 http://dx.doi.org/10.3390/ijerph19138001

Surveillance of swine influenza virus in Hanoi, Vietnam, during 2013-2019 revealed gene pool enrichment from imported swine from Asia and North America and showed long-term maintenance, persistence, and reassortment of virus lineages. Genome sequencing showed continuous enrichment of H1 and H3 diversity through repeat introduction of human virus variants and swine influenza viruses endemic in other countries. In particular, the North American H1-d1a strain, which has a triple-reassortant backbone that potentially results in increased human adaptation, emerged as a virus that could pose a zoonotic threat. Vu, H; Cheong, IH; Luong, JHC; Simeon-Dubach, D; Le, D; Pham, THX; Thanh, NT; Dat, NQ Vietnam biobanking feasibility study: An overview of biobanking landscape, infrastructure, and capacity 越南生物样本库可行性研究:生物样本库现状、基础设施与能力概览 AIMS PUBLIC HEALTH Within Southeast Asia, there are still some challenges in managing non-communicable diseases and public health. Vietnam's government has supported its biotech research and pharmaceutical capacity for many years, including creating research infrastructure such as biobanks. To better understand the situation of biobanks in Vietnam, a feasibility study adapted from previous biobank work in Southeast Asia was conducted to analyze the knowledge gap for biobanking in Vietnam. The questionnaire was redefined for the purpose of the Vietnamese research climate, with 66 items categorized into 8 sections. Specifically, the study analyzed the following information: 1) General Information, (2) Biobank Information, (3) Sample and Storage Management, (4) Biobank Infrastructure, (5) Operational Resources and Personnel, (6) Laboratory Information Management Systems (LIMS), (7) Quality Management Systems (QMS), and (8) Risk Management Systems. The results showed that most biobanks in Vietnam have good energy supplies and backup power. Internet access was more critical, and backup sources were rare. Human resource management and training programs were well established. The QMS and LIMS were widespread, thus promoting confidence in the quality of biobanking, while waste management was also in place. However, reliable and available dry ice was hard to find, and sensitive data/documents lacked confidentiality. Most biobanks have a contingency plan, but alert systems/breach procedures raised questions about the quality of the contingency plans. To our knowledge, this is the first feasibility study of the biobank infrastructure in Vietnam. Our results, based on a selected group of biobanks, suggest there are well-established; however, there are critical challenges that should be addressed in the near future. Ultimately, the findings in this study should provide a foundation for future policy work, informed strategic planning, and resource allocation for the sustainable development and operation of biobanks in Vietnam. Liu, HB; Xiao, WW; Du, XY; Xue, JZ; Wang, H; Wang, Q; Wang, YL; Jia, HQ; Song, HB; Qiu, SF Molecular Detection of Tick-Borne Bacterial Pathogens in Ticks and Rodents from the China-Vietnam Border 中越边境地区蜱及啮齿动物体内蜱传细菌病原体的分子检测 VETERINARY SCIENCES Ticks and tick-borne diseases constitute a crucial focus for the health of both humans and animals worldwide. Although numerous studies on tick-borne diseases have been conducted in China, reports on tick-borne pathogens in ticks and rodents from the China-Vietnam border are scarce. In this study, we investigated tick-borne bacterial pathogens, including Rickettsia, Anaplasmataceae, and Borrelia, in nine rodents (Rattus norvegicus) and 88 ticks collected from cattle and rodents in Jingxi, a city at the China-Vietnam border. Through molecular detection and sequence analysis, four known tick-borne pathogens were identified. Specifically, Rickettsia japonica was detected in 46.3% (37/80) of Haemaphysalis cornigera; Anaplasma phagocytophilum and Candidatus Neoehrlichia mikurensis were identified in one Ixodes granulatus and one rodent, respectively; and Borrelia valaisiana was detected in two I. granulatus. Additionally, a potentially novel species of Rickettsia, provisionally named Rickettsia sp. JX, was detected in 41.3% (33/80) of Ha. cornigera, one Rhipicephalus microplus, three I. granulatus, and nine rodents, whereas a potentially novel species of Borrelia, tentatively named Borrelia sp. JX, was detected in one I. granulatus. To the best of our knowledge, this is the first report on tick-borne bacterial pathogens in ticks and rodents from the China-Vietnam border. These results expand the knowledge of the geographical distribution and vector diversity of tick-borne bacterial pathogens in China and are conducive to the evaluation of thee potential public health risk. 本研究旨在调查中越边境地区(广西靖西市)蜱虫和啮齿动物中携带的蜱传细菌性病原体。通过对牛和啮齿动物采集的蜱虫及啮齿动物样本进行分子检测与序列分析,共鉴定出四种已知的蜱传病原体,包括在日本血蜱中检出的日本立克次体、在颗粒血蜱和啮齿动物中分别检出的嗜吞噬细胞无形体和米库尔新埃立克体,以及在颗粒血蜱中检出的瓦莱疏螺旋体。此外,还发现了一种暂命名为立克次体JX的潜在新种和一种暂命名为疏螺旋体JX的潜在新种。这是中越边境地区蜱虫和啮齿动物中蜱传细菌性病原体的首次报道,研究结果扩展了相关病原体的地理分布和媒介多样性认知,有助于评估潜在的公共卫生风险。 2025 http://dx.doi.org/10.3390/vetsci12030256
Curran, K; Congdon, N; Hoang, TT; Lohfeld, L; Nguyen, VT; Nguyen, HT; Nguyen, QN; Dardis, C; Virgili, G; Piyasena, P; Tran, H; Salongcay, RP; Tung, MQ; Peto, T Impact of targeted diabetic retinopathy training for graders in Vietnam and the implications for future diabetic retinopathy screening programmes: a diagnostic test accuracy study 越南分级员糖尿病视网膜病变针对性培训的效果及其对未来筛查项目的影响:一项诊断试验准确性研究 BMJ OPEN Objectives To compare the accuracy of trained level 1 diabetic retinopathy (DR) graders (nurses, endocrinologists and one general practitioner), level 2 graders (midlevel ophthalmologists) and level 3 graders (senior ophthalmologists) in Vietnam against a reference standard from the UK and assess the impact of supplementary targeted grader training. Design Diagnostic test accuracy study. Setting Secondary care hospitals in Southern Vietnam. Participants DR training was delivered to Vietnamese graders in February 2018 by National Health Service (NHS) UK graders. Two-field retinal images (412 patient images) were graded by 14 trained graders in Vietnam between August and October 2018 and then regraded retrospectively by an NHS-certified reference standard UK optometrist (phase I). Further DR training based on phase I results was delivered to graders in November 2019. After training, a randomised subset of images from January to October 2020 (115 patient images) was graded by six of the original cohort (phase II). The reference grader regraded all images from phase I and II retrospectively in masked fashion. Primary and secondary outcome measures Sensitivity was calculated at the two different time points, and chi(2) was used to test significance. Results In phase I, the sensitivity for detecting any DR for all grader groups in Vietnam was low (41.8-42.2%) and improved in phase II after additional training was delivered (51.3-87.2%). The greatest improvement was seen among level 1 graders (p<0.001), and the lowest improvement was observed among level 3 graders (p=0.326). There was a statistically significant improvement in sensitivity for detecting referable DR and referable diabetic macular oedema between all grader levels. The post-training values ranged from 40.0 to 61.5% (including ungradable images) and 55.6%-90.0% (excluding ungradable images). Conclusions This study demonstrates that targeted training interventions can improve accuracy of DR grading. These findings have important implications for improving service delivery in DR screening programmes in low-resource settings. 本研究旨在评估越南不同级别糖尿病视网膜病变分级人员的分级准确性,并评估针对性补充培训的效果。研究分为两个阶段:第一阶段,经英国国家卫生服务体系培训的越南分级人员对眼底图像进行分级,结果显示所有级别分级人员对任一视网膜病变的检测灵敏度均较低。第二阶段,基于第一阶段结果进行额外培训后,分级灵敏度显著提高,其中一级分级人员(护士、内分泌科医生等)的提升最为显著,三级分级人员(资深眼科医生)的提升不显著。对于可转诊的糖尿病视网膜病变及糖尿病性黄斑水肿的检测灵敏度,所有级别分级人员均有统计学意义的改善。研究结论表明,针对性的培训干预能够提高糖尿病视网膜病变分级的准确性,对在资源有限环境中改善筛查服务具有重要启示。 2022 http://dx.doi.org/10.1136/bmjopen-2021-059205

This study analyzed and compared the genomic sequences of ASF viruses (ASFVs) during the first outbreak in Hung Yen (VN/HY/2019-ASFV1) and Quynh Phu provinces (VN/QP/2019-ASFV1) in Vietnam in 2019, and the subsequent outbreak in Hung Yen (VN/HY/2022-ASFV2) in 2022, to those of other ASFV strains. VN/HY/2019-ASFV1, VN/QP/2019-ASFV1, and VN/HY/2022-ASFV2 genomes were 189,113, 189,081, and 189,607 bp in length, encoding 196, 196, and 203 open reading frames (ORFs), respectively. VN/HY/2019-ASFV1 and VN/QP/2019-ASFV1 shared a 99.91-99.99% average nucleotide identity with genotype II strains. Variations were identified in 28 ORFs in VN/HY/2019-ASFV1 and VN/QP/2019-ASFV1 compared to 20 ASFV strains, and 16 ORFs in VN/HY/2022-ASFV2 compared to VN/HY/2019-ASFV1 and VN/QP/2019-ASFV1. Vietnamese ASFV genomes were classified as IGR II variants between the I73R and I329L genes, with two copy tandem repeats between the A179L and A137R genes. A phylogenetic analysis based on the whole genomes of 27 ASFV strains indicated that the Vietnamese ASFV strains are genetically related to Estonia 2014, ASFV-SY18, and Russia/Odintsovo_02/14. These results reveal the complete genome sequences of ASFV circulating during the first outbreak in 2019, providing important insights into understanding the evolution, transmission, and genetic variation of ASFV in Vietnam. 本研究对越南2019年首次暴发及2022年再次暴发的非洲猪瘟病毒株进行了全基因组测序与比较分析。结果显示,2019年两株病毒序列长度相近,编码开放阅读框数量相同;2022年毒株序列略长,编码开放阅读框数量增加。2019年毒株与基因型II参考株的核苷酸同源性极高。与多株参考株相比,2019年毒株在多个开放阅读框中存在变异;2022年毒株相较于2019年毒株也有多处开放阅读框变异。越南毒株均被归类为IGR II变异型,并在特定基因间存在双拷贝串联重复序列。全基因组系统发育分析表明,越南毒株与爱沙尼亚、俄罗斯及中国SY18株亲缘关系较近。这些发现为理解非洲猪瘟病毒在越南的进化、传播及遗传变异提供了重要依据。 2023 http://dx.doi.org/10.3390/v15091945
