The annual parasite index (API) exceeds 1 per 10,000 population in the northern seven townships of Rakhine State, with Minbya Township designated as a highburden area for malaria by the World Health Organization (WHO) Myanmar and the National Malaria Control Programme (NMCP). Since 2017, the Malaria reduction intensification plan has been in place in this township to combat the high disease transmission rates. This study aims to assess the malaria epidemiology in Minbya Township from 2017 to 2020 under the intensification plan for elimination, as well as to evaluate the effectiveness of the integrated strategy in reducing cases in hotspot areas. Methods : The study utilized a surveillance study design to collect secondary data from the Malaria surveillance system (MSS) and the epidemiologic monitoring dashboard of Minbya Township, Maruk-U District, located in Rakhine State. Results : Since 2017, the Malaria prevention and control (P&C) Program in Minbya Township has successfully decreased malaria morbidity, eliminated malaria -related deaths, and bolstered malaria testing capabilities through the participation of village health volunteers (VHVs). Approximately 87 % of malaria prevention and control services are executed by the township's malaria elimination and disease control programs, with additional support from stakeholders. The API dropped from 13 in 2017 to 2.5 in 2020, with Plasmodium vivax being the most prevalent malaria species, accounting for 55 % of cases. However, there still lacks robust evidence of imported malaria caused by Plasmodium spp. infected anopheles along international border areas (border-spill malaria). The objectives of this study were to confirm whether an outbreak of Plasmodium vivax malaria is border-spill malaria and assess the effects of China's public health response along China-Myanmar border. Methods Epidemiological, parasitological and entomological investigations were conducted to investigate the outbreak of border-spill malaria. Meanwhile, comprehensive interventions were carried out to prevent further transmission and reintroduction of malaria. Results Rapid diagnostic testing, microscopy and polymerase chain reaction were performed and the infections were confirmed as P. vivax. A total of 22 (9.21%) of 239 workers contracted P. vivax during the outbreak. Multivariate logistic regression analysis identified that the distance of worker shelters in China within 300 meters to the internally displaced person (IDP) camps in Myanmar was a risk factors associated with malaria infection (adjusted odds ratio 7.5920; 95% confidence interval, 2.6079-22.1013; P = 0.0002). After comprehensive interventions, malaria transmission was successfully interpreted and prevented at the project site till the completion of project on 14 January 2020, and recurrence of P. vivax malaria was not detected by the end of 2020. Conclusion This study provided robust evidence of border-spill malaria along China-Myanmar border. Malaria parasite reservoir and distance travelled by female anopheline mosquitoes are two determinants for border-spill malaria. The public health response to the outbreak indicates that the malaria surveillance and response system works well in preventing reintroduction of malaria. Myanmar is endowed with diverse vegetable species and many are also used as traditional medicines. In this study, we investigated 48 vegetable samples from central Myanmar for their inhibitory activities against the type III secretion system (T3SS) of Salmonella enterica serovar Typhimurium, one of the most prevalent pathogenic bacteria that causes infections. Results showed that 15 vegetables exhibited moderate to strong inhibitory activities against the secretion of T3SS effector proteins, providing the scientific evidence for the traditional knowledge of vegetables and their potential pharmacological function. By detailed chemical investigation, we found the vegetables, i.e., Archidendron jiringa (Jack) Nielsen., Neptunia plena (Linn.) Benth. and Ludwigia adscendens (L.) Hara have the potential for discovering the components with anti-T3SS activity. Bioassay-guided isolation of L. adscendens led to identify arjunglucoside II as anew T3SS inhibitor. However, there is an eminent lack of information on the attitudes and traditional beliefs of local farmers and livestock supply chain actors in CDZ of Myanmar on the public health implications. A modified data collection instrument of the Health Belief model was developed to investigate attitudes, beliefs and barriers to the application of recommended zoonotic disease prevention.Study designCross-sectional study.MethodData analyses were conducted considering a two-phase multilevel mixed effect binomial generalized linear models modelling approach.ResultsThe availability of information about zoonosis to supply chain actors influenced their confidence to implement preventive actions (OR = 1.5, p = 0.045 for cattle diseases; OR = 1.5, p = 0.022 for village chicken diseases). Supply chain actors were more likely aware of zoonosis transmitted by cattle compared to livestock farmers (OR = 0.3, p = 0.005 for cattle farmers), while people not rearing or trading small ruminants and/or poultry were less likely to be aware of the zoonotic risk associated with these animals (p < 0.005). Information on zoonosis transmitted from small ruminants was mainly promoted through farmers (p = 0.032), while information on zoonotic diseases that can be obtained from chickens was disseminated through farmers, local authorities and the media. During the 2019 dengue outbreak in Yangon, Myanmar, molecular data on CHIKV were limited among dengue-suspected patients and there were no publicly available CHIKV genome sequences from Yangon in international databases. To address this gap and potential diagnostic overlap, we investigated the prevalence of CHIKV infection and described the genomic characteristics of detected strains.Methods Serum samples from 267 dengue-suspected patients collected in 2019 were screened for anti-CHIKV IgM and IgG by in-house ELISA and 211 samples with sufficient remaining volume were further analyzed by RT-qPCR, isolation of the virus, and whole-genome sequencing for mutation analysis.Results CHIKV antibodies were found in 24.7% (66/267) of samples (IgM 3.4%, IgG 21.3%), and viral RNA was detected in 10.9% (23/211) of samples. Fifteen viral isolates were successfully obtained (7.1% of those tested), including two co-detections with DENV-2 by RT-PCR. All isolates belonged to the East/Central/South African genotype, Indian Ocean Lineage (ECSA-IOL), and clustered with strains from Thailand, China, and Mandalay, Myanmar. Whole-genome analysis identified 33 non-synonymous mutations across nonstructural and structural proteins, including mutations previously reported in regional ECSA-IOL strains such as E1:K211E and E2:V264A, with 11 amino acid changes not previously reported in available Myanmar reference sequences.Discussion Serological and molecular findings indicate CHIKV circulation during the 2019 dengue outbreak in Yangon and highlight the limitations of single-target testing. Serological evidence indicate the presence of anti-CHIKV IgM and IgG antibodies, reflecting CHIKV exposure within the study population. Notably, all RNA-positive cases were seronegative for both IgM and IgG, a pattern consistent with the temporal dynamics of infection and the inherent constraints of serological detection in co-endemic settings. We genotyped 125 P vivax clinical samples at two merozoite surface protein loci, PvMSP1 and PvMSP3 alpha, by amplicon deep sequencing. Our study revealed that the parasite population in this region was highly diverse, identifying 60 PvMSP1 and 98 PvMSP3 alpha haplotypes, with haplotype diversity of 0.929 and 0.944, respectively. Remarkably, 97.6 % (122/125) of the patients harbored multiclonal infections with a mean multiplicity of infection of 4.18, indicating a relatively high transmission intensity. Neutrality tests and network analysis suggested a recent parasite population expansion, consistent with the concurrent malaria outbreak in the region. This study assessed the spatio-temporal dynamics and environmental predictors of Plasmodium falciparum and Plasmodium vivax malaria in Myanmar. Methods Monthly reports of malaria cases at primary health centers during 2011-2017 were analyzed to describe malaria distribution across Myanmar at the township and state/region levels by spatial autocorrelation (Moran index) and spatio-temporal clustering. Negative binomial generalized additive models identified environmental predictors for falciparum and vivax malaria, respectively. Results From 2011 to 2017, there was an apparent reduction in malaria incidence in Myanmar. Malaria incidence peaked in June each year. There were significant spatial autocorrelation and clustering with extreme spatial heterogeneity in malaria cases and test positivity across the nation (P < 0.05). Areas with higher malaria incidence were concentrated along international borders. Primary clusters of P. falciparum persisted in western townships, while clusters of P. vivax shifted geographically over the study period. The primary cluster was detected from January 2011 to December 2013 and covered two states (Sagaing and Kachin). Annual malaria incidence was highest in townships with a mean elevation of 500-600 m and a high variance in elevation (states with both high and low elevation). There was an apparent linear relationship between the mean normalized difference vegetative index and annual P. falciparum incidence (P < 0.05). Conclusion The decreasing trends reflect the significant achievement of malaria control efforts in Myanmar. Therefore, we aimed to assess the population mental health burden, healthcare needs, and the associated risk factors in Myanmar. Methods We established a nationwide random sample and recruited 1038 adults via random digit dialling from July 3 - Aug 9, 2021, during the ongoing con fl ict since Feb 1, 2021, and surge in SARS-CoV-2 infections. Probable post -traumatic stress disorder (PTSD) was assessed using the PTSD Checklist - Civilian Version. Probable depression and anxiety were assessed using the Patient Health Questionnaire -2 and the Generalized Anxiety Disorder -2. We calculated population attributable fractions for probable mental disorders using multivariable logistic regression models. Based on the mental health burden and healthcare -seeking patterns, we projected the need for mental health services. Findings During the ' triple crises ' , a third of adults in Myanmar (34.9%, 95% CI 32.0 - 37.7) reported a probable mental disorder. Prevalence of probable PTSD, depression, and anxiety were 8.1% (6.6 - 9.7), 14.3% (12.0 - 16.6), and 22.2% (19.7 - 24.7), respectively. We estimated that up to 79.9% (43.8 - 97.9) of probable PTSD was attributable to political stress. This corresponds to 2.1 million (1.1 - 3.2 million) fewer adults with probable PTSD if political stress was removed from the population. The mental health burden could translate into roughly 5.9 million adults seeking mental health services. Interpretation The mental health burden in Myanmar is substantial, and population mental health might only be restored when the three crises have ended. An accelerated peace process is critical to protecting Myanmar ' s population mental health. Methods: E. coli LD91-1 was subjected to antimicrobial susceptibility testing. Extracted genomic DNA from the E. coli LD91-1 clone was sequenced using a Nanopore PromethION sequencer (ONT, UK) and the highthroughput Illumina HiSeq 2500 platform. The clean reads were de novo assembled using Flye (v.2.9). The complete genome was annotated using MLST, phylogeny, serotyping, plasmid replicons, fimH typing, chromosomal point mutations, antibiotic resistance, and virulence genes using web tools available at the Center for Genomic Epidemiology. Results: E. coli LD91-1 had a circular genome of 4,838,891 bp. In silico analysis revealed that E. coli LD91-1 comprised plasmid pLD91-1 and belonged to the ST1737-O22:H16 serotype carrying a fimH32 variant that had not been reported in such strains. Core genome phylogeny demonstrated clear divergence among major E. coli clades. Mutations in the quinolone resistance-determining regions gyrA, gyrB, parC, and parE conferred fluoroquinolone resistance; mutations in pmrA and pmrB conferred colistin resistance. Cephalosporin, fluoroquinolone, and tetracycline resistance genes were located on plasmid pLD91-1. Notably, several virulence genes, including 12 encoding yersiniabactin and yersiniabactin siderophores were also revealed in the E. coli LD91-1 genome. Conclusions: To our knowledge, this is the first complete genome report of a new E. coli LD91-1 carrying the fimH32 variant in Myanmar.
